The P.A.D. Coalition is proud to annouce a “call for nominations” for the 2008 Stay in Circulation Community Service Award. Below are award details and application instructions.
Purpose
To recognize collaborative programs focused on increasing awareness about peripheral arterial disease (P.A.D.) to high-risk populations, patients and/or the health care community.
Award Categories
One award in each of the following categories will be awarded annually:
Local: To recognize local community groups, hospitals, professional chapters and other entities whose efforts are targeted to a local or regional area.
Criteria
Potential nominees will be judged by several criteria, which include:
Numbers of persons reached
Originality and innovative design
Implementation and evaluation
Media outreach efforts, if appropriate
Distribution and use of Staying in Circulation: Take Steps to Learn about P.A.D. campaign materials and messages
Added value will be awarded to those programs that can demonstrate the following:
Eligibility
Any member of a P.A.D Coalition member organization can submit nominations for this award. The nominated program must have been implemented between July 1, 2007 and July 1, 2008 and used the messages, concepts and/or materials presented in the national Stay in Circulation: Take Steps to Learn about P.A.D. campaign.

The deadline for receipt of entries for the 2008 awards is July 25, 2008. Awardees will be contacted by Aug 11, and the award will be presented at the P.A.D. Coalition’s Annual Meeting in Washington D.C., September 8, 2008.
Review
Nominated entrees will be reviewed by an independent, multidisciplinary panel of judges representing P.A.D. Coalition member organizations. Members of the committee and their organizations will not be eligible for this award.
Award Winner
Award winners receive a plaque and up to $1,000 in travel expenses to attend the P.A.D. Coalition’s Annual Meeting to accept the award. The award winners will be promoted through a national press release and local media activities, as appropriate.
