The P.A.D. Atlas:


Developed in partnership with the National Minority Quality Forum (NMQF), the P.A.D. Atlas is a database that maps by zip code P.A.D. prevalence down to the street level.  Researchers, patient advocacy groups, legislators and allied health professionals can use the P.A.D. Atlas to identify prevalence of P.A.D. at the national, state, and local
levels to direct educational resources where they are needed most.

Updated P.A.D. Clinician Toolkit Now Available:

 

In cooperation with the P.A.D. Coalition, the American Heart Association has updated its P.A.D. Clinician Toolkit to help health professionals integrate the intersocietal P.A.D. guidelines into practice. This kit includes a condensed, easy-to-read reference on the intersocietal P.A.D. guidelines, an instructional DVD on conducting the ankle-brachial index (ABI), and patient education materials. P.A.D. Wall Chart Available for Medical Offices:

The American College of Cardiology in cooperation with the P.A.D. Coalition has produced a laminated wall chart for medical offices. Targeted to patients, this wall chart (24” x 19”) features graphics of the vascular system and reviews P.A.D. risk factors, symptoms, diagnostic tests, and treatment. Patient Education Resources Available from Member Organizations

PCNA Forms: Practical Information for your Cardiovascular Risk Reduction Clinic- Medical Evaluation Forms, Assessment Tools, and Educational Material: Contains PDF templates of clinical forms and patient handouts that can be used in CVD risk reduction practice.  These forms may be personalized with patient and clinic/provider information and can be integrated into a patient’s medical record.  This tool includes patient assessment forms, contracts, flow charts, follow-up plans & tools, patient-education materials, and a list of online resources.  Forms are available (free of charge) online at www.pcna.net.
